Restless Leg Syndrome Treatment in Adults with End-Stage Renal Disease

Adults aged 18 and older with restless legs syndrome (RLS) who also have end-stage renal disease (ESRD) represent a distinct clinical population. The coexistence of ESRD shapes treatment selection and the range of options available.

This protocol applies to adult patients (18 years or older) with restless legs syndrome in the setting of end-stage renal disease.

Treatment Approach

The AASM-guided approach for this population includes specific pharmacological therapy. Gabapentin is one of the recommended options in this setting; additional interventions may apply depending on the patient's laboratory profile.

  • In adults with RLS and ESRD, the AASM suggests the use of gabapentin over no gabapentin (conditional recommendation, very low certainty of evidence).
  • In adults with RLS and ESRD, the AASM suggests the use of IV iron sucrose over no IV iron sucrose in patients with ferritin < 200 ng/mL and transferrin saturation < 20% (conditional recommendation, moderate certainty of evidence).
