This protocol addresses renal vein thrombosis presenting as part of venous thromboembolism (VTE) in a patient with active cancer, specifically excluding gastrointestinal or genitourinary malignancy. The presence of active cancer substantially shapes the anticoagulation strategy.
Oral factor Xa inhibitor therapy is recommended as part of the first-line approach for cancer-associated VTE in this population. The specific agent selection and full management algorithm are available in the structured protocol below.

In patients with cancer-associated VTE, oral factor Xa inhibitors (apixaban, edoxaban, rivaroxaban) are recommended over LMWH or vitamin K antagonists because of the oral administration of DOACs and their safety and efficacy without the need for laboratory monitoring.
