Renal sarcoidosis with hypercalciuria or mild hypercalcemia (serum calcium ≤11 mg/dL, no nephrolithiasis) — first-line management

When renal sarcoidosis presents with elevated urine calcium or mildly elevated serum calcium that has not yet reached the threshold for aggressive intervention, and nephrolithiasis is absent, a defined first-line approach applies before escalation is considered.

This protocol addresses patients with renal sarcoidosis who have hypercalciuria or mild hypercalcemia — serum calcium at or below 11 mg/dL — and no current nephrolithiasis. The presence of elevated serum or urinary calcium in this range, without stone disease, defines the population for this initial management step.

The therapeutic target is a measurable reduction in urine calcium or serum calcium toward normal range.

In isolated hypercalciuria, treatment may begin with a reduction in calcium intake, increased fluids and avoidance of sun.

Mild hypercalcemia may also be treated with a reduction in dietary calcium and increased fluid intake.
