Treatment of Metastatic Clear-Cell Renal Cell Carcinoma in IMDC Intermediate- or Poor-Risk Disease
Patients with metastatic clear-cell renal cell carcinoma (ccRCC) classified as intermediate- or poor-risk by IMDC criteria represent a distinct population whose risk stratification directly determines the recommended treatment approach across all lines of therapy.
Metastatic clear-cell renal cell carcinoma with IMDC intermediate- or poor-risk classification. The IMDC risk category is a primary driver of agent selection and sequencing decisions throughout the treatment course.
For patients who have progressed after prior lines of therapy, third-line management involves sequencing from agents not previously used, with further alternatives available for those treated across multiple prior lines.
Offer nivolumab plus ipilimumab, pembrolizumab plus axitinib, lenvatinib plus pembrolizumab or nivolumab and cabozantinib to patients with International Metastatic Renal Cell Carcinoma Database Consortium (IMDC) intermediate- or poor-risk disease.
Sequencing the agent not used as second-line therapy (nivolumab or cabozantinib) for third-line therapy is recommended.
Offer belzutifan as an alternative to everolimus in patients previously treated with second- to fourth-line therapy for clear cell metastatic RCC.
