This protocol addresses metastatic clear-cell renal cell carcinoma in patients classified as intermediate- or poor-risk by the International Metastatic Renal Cell Carcinoma Database Consortium (IMDC) criteria. IMDC risk stratification directly determines the recommended treatment pathway for this population.

Offer nivolumab plus ipilimumab, pembrolizumab plus axitinib, lenvatinib plus pembrolizumab or nivolumab and cabozantinib to patients with International Metastatic Renal Cell Carcinoma Database Consortium (IMDC) intermediate- or poor-risk disease.
Offer cabozantinib or other VEGF-TKI as second-line therapy to patients refractory to nivolumab plus ipilimumab or axitinib plus pembrolizumab or cabozantinib plus nivolumab or lenvatinib plus pembrolizumab.
