First-Line Treatment for Metastatic Clear-Cell Renal Cell Carcinoma — IMDC Intermediate- or Poor-Risk
Clinical Scenario
This protocol applies to patients presenting with metastatic clear-cell renal cell carcinoma (ccRCC) who are classified as intermediate- or poor-risk according to the International Metastatic Renal Cell Carcinoma Database Consortium (IMDC) criteria and are candidates for first-line systemic therapy.
Patient Population
The IMDC intermediate- and poor-risk categories encompass a broad group within metastatic ccRCC for whom specific first-line treatment selection is addressed by current guidelines. This is not a favourable-risk scenario; the protocol is distinct to this risk tier.

- Offer nivolumab plus ipilimumab, pembrolizumab plus axitinib, lenvatinib plus pembrolizumab or nivolumab and cabozantinib to patients with International Metastatic Renal Cell Carcinoma Database Consortium (IMDC) intermediate- or poor-risk disease.
- Offer sunitinib or pazopanib to patients with any IMDC risk who cannot receive or tolerate immune checkpoint inhibition (ICI).
- Offer cabozantinib to patients with IMDC intermediate- and poor-risk clear cell metastatic renal carcinoma (cc-mRCC) who cannot receive or tolerate ICI.
