Treatment of Pulmonary Arteriovenous Malformation in Patients Not Allergic to Penicillin

Clinical scenario

This protocol addresses the management of pulmonary arteriovenous malformation (PAVM) in patients who have no known allergy to penicillin. Penicillin-allergy status is a key determinant of antibiotic selection in this condition: coamoxiclav is the preferred agent for patients without penicillin allergy, while different agents are used in those who are allergic.

Treatment approach (partial)

Patients with PAVMs require prophylactic antibiotic cover before dental, endoscopic, surgical, and other interventional procedures that carry a risk of transient bacteraemia. In patients who are not allergic to penicillin, a specific oral antibiotic regimen — given before and after the procedure — is recommended.

For patients with PAVMs, judicious dental hygiene is recommended, together with prophylactic antibiotics prior to dental, endoscopic, surgical and other interventional procedures that could lead to transient bacteraemias.

Coamoxiclav is the preferred agent with clindamycin or metronidazole suggested for penicillin-allergic patients.
