Treatment of Pulmonary Arteriovenous Malformation in Penicillin-Allergic Patients

Patients with pulmonary arteriovenous malformation (PAVM) face an elevated risk of haematogenous infection during invasive procedures. When the patient carries a penicillin allergy, the standard prophylactic agent cannot be used, and a specific alternative approach is required.

Clinical Scenario

This protocol applies to PAVM in the setting of confirmed or reported penicillin allergy. Because co-amoxiclav — the preferred first-line prophylactic agent — is contraindicated, a distinct antibiotic selection is indicated for this sub-population prior to invasive procedures.

Approach

Management centres on prophylactic antibiotic cover before dental, endoscopic, surgical, and other invasive procedures that could lead to transient bacteraemias. For penicillin-allergic patients, alternative antibiotic agents are prescribed in place of the standard agent.
