Treatment of Psoriasis Involving the Face or Inverse (Intertriginous) Areas

Psoriasis affecting the face or skin folds presents a distinct clinical challenge. These sensitive sites require a carefully selected topical approach — the standard considerations that apply to plaque psoriasis elsewhere do not apply equally here.

This protocol is for patients with psoriasis involving the face, or inverse (intertriginous) psoriasis — areas where skin sensitivity and the risk of local adverse effects, including atrophy, constrain treatment selection.

Treatment Approach

Topical corticosteroid therapy forms the basis of treatment, but the specific potency tier used for the face and intertriginous areas differs from what is used elsewhere. The full regimen — including agent selection, sequencing, and alternative options — is in the structured protocol.

Lower potency corticosteroids should be used on the face, intertriginous areas, and areas that are susceptible to steroid atrophy (eg, forearms) and other adverse effects.

The off-label use of 0.1% tacrolimus for psoriasis involving the face as well as inverse psoriasis for up to 8 weeks can be considered.
