Psoriasis in Obesity (BMI ≥30 kg/m²): Next Step When Standard Weight Loss Measures Fail

This protocol is for psoriasis patients who also have obesity — a body mass index of 30 kg/m² or higher — and who have not achieved adequate clinical targets on an initial dietary weight loss intervention.

Clinical scenario

Psoriasis with Obesity (BMI ≥30 kg/m²)

Patients with moderate-to-severe psoriasis should have their obesity status assessed annually. Those with a BMI above 30 kg/m² or with abdominal obesity require referral for further evaluation and management — obesity is a clinically significant comorbidity that directly influences psoriasis treatment outcomes.

Previous therapy — insufficient response

When a Low-Energy Diet Does Not Reach Targets

The prior treatment step — a low-energy dietary intervention used as a standard weight loss measure — aims to achieve a modest reduction in psoriasis severity (Psoriasis Area Severity Index), meaningful weight loss, and improvement in quality of life (Dermatology Life Quality Index). When these goals are not met, escalation to the next protocol is indicated.

For eligible psoriasis patients who have not responded adequately to standard weight loss measures, this protocol addresses a bariatric surgical approach to weight reduction. Patients with moderate-to-severe psoriasis should have their obesity status determined annually.

Patients of obesity class 1 or higher (BMI >30 kg/m²) and/or abdominal obesity should be referred to their primary care physician for further education and evaluation.

Thus, if a psoriasis patient with a BMI >40 kg/m² fails standard weight loss measures, gastric bypass surgery should be strongly considered.

Bariatric surgery should be considered to improve the comorbidities in psoriasis patients with a body mass index >40 kg/m² who fail standard weight loss measures.
