Treatment of Psoriasis in Nail Psoriasis

This protocol is indicated for psoriasis presenting with nail psoriasis — a localised form of psoriatic disease in which the nails are the primary site of involvement.

Nail psoriasis represents a specific clinical situation within psoriatic disease. Off-label topical therapy is the documented first-line treatment strategy for this localised presentation, with evidence supporting both monotherapy and combination approaches applied directly to the affected nails.

Treatment approach — partial overview

An off-label topical retinoid — used alone or in combination with a topical corticosteroid — is applied to the nails. The complete sequence, application details, and clinical decision points are contained in the full structured protocol.

The off-label use of topical tazarotene may be recommended as monotherapy or in combination with topical corticosteroids for the treatment of localized pediatric skin or nail psoriasis.

Daily application of tazarotene 0.05% gel to the nails for 8 weeks afforded improvement of the nail disease, but local irritation occurred.
