Generalized Plaque Psoriasis in Adults When NB-UVB Combination Therapy Has Not Achieved Adequate Clearance

Adults (age 18 and older) with generalized plaque psoriasis sometimes do not reach sufficient skin clearance with combination NB-UVB phototherapy. This protocol defines the next clinical step for that situation.

Previous treatment & reason for escalation

The prior regimen consisted of NB-UVB phototherapy three times weekly combined with systemic or topical adjuncts. Despite this approach, the key clearance targets were not met: neither a 75% improvement in the Psoriasis Area and Severity Index (PASI 75) nor a 90% improvement (PASI 90) was achieved.

NB-UVB phototherapy — wavelengths from 311 to 313 nm — is a recommended option for adults with plaque psoriasis; when it falls short of these thresholds, a further treatment step is indicated.

Treatment direction (partial overview)

The next step uses an oral photosensitizing strategy: a sensitizing agent taken by mouth followed by controlled UVA light exposure. This approach may be combined with an oral retinoid to enhance efficacy. Specific dosing, scheduling, skin-type adjustments, and the complete algorithm are provided in the structured protocol.
