This protocol addresses psoriasis management in patients whose oral systemic nonbiologic monotherapy did not produce sufficient skin clearance after an adequate treatment course.
Prior therapy: oral systemic nonbiologic monotherapy — including agents such as methotrexate, apremilast, acitretin, tofacitinib, or fumaric acid esters used as a single agent.
Escalation trigger: failure to achieve PASI 75 (a 75% improvement in the Psoriasis Area and Severity Index) or clear / almost clear skin after 16 weeks of treatment.
More rapid skin clearing, with clear or almost clear skin as the target outcome.

Combination therapy with methotrexate and NB-UVB phototherapy can be considered for adult patients with generalized plaque psoriasis to enhance efficacy and lower cumulative doses of both treatments.
Methotrexate has also been used with NB-UVB phototherapy. This combination results in increased efficacy and more rapid skin clearing at lower cumulative doses of methotrexate and UVB compared with monotherapy with either treatment.
