Low-Risk Localised Prostate Cancer with Life Expectancy Under 10 Years
This protocol applies to men with low-risk, localised prostate cancer — ISUP grade group 1, PSA below 10 ng/mL, and clinical stage cT1–2a — whose life expectancy is less than 10 years.
When a patient's life expectancy is under 10 years due to age or comorbidities, curative treatment would not be a realistic option in the event of disease progression. This subset of men with otherwise low-risk, localised disease requires a management approach tailored to that overall prognosis. Watchful waiting is the standard of care in this setting.
The strategy in this scenario centres on watchful waiting, with palliative treatment considered only once symptoms arise — rather than immediate or curative-intent intervention. The complete protocol detailing the criteria and next steps is available via the link below.
For patients with a life expectancy of < 10 years (based on comorbidities and age) for whom curative treatment would not be an option in the case of progression after AS, WW is standard of care.
