Treatment of Prolactinoma in Females Who Are Pregnant or Desiring Pregnancy

Females with prolactinoma who are pregnant or planning a pregnancy represent a specific clinical population whose management requires careful consideration of both reproductive goals and adenoma behaviour.

Clinical Scenario

This protocol applies to females with prolactinoma who are currently pregnant or who desire pregnancy. Patients in this situation should be informed about both medical and surgical treatment options, as the appropriate choice depends on individual clinical circumstances.

Approach

When the adenoma does not respond as expected to resumed medical management, surgical intervention is one of the options that may need to be considered. The complete decision framework — including which intervention applies and under what conditions — is contained in the full structured protocol.

  • Patients with prolactinoma considering pregnancy should be informed about both medical and surgical options (strong).
  • Most workshop participants recommended medical treatment with dopamine agonists as the first choice of therapy for women with prolactinoma desiring pregnancy.
  • In patients with an enlarged adenoma that does not respond to re-initiation of dopamine agonist therapy, consideration should be given to pituitary surgery or delivery if the pregnancy is sufficiently advanced (strong).
