This protocol applies to premenopausal and postmenopausal women diagnosed with a microprolactinoma who do not desire pregnancy. Because fertility is not a treatment goal, the management pathway differs substantially from approaches used in women who wish to conceive.
Among premenopausal women with microprolactinoma and no pregnancy goal, gonadal function status is a key factor that shapes the appropriate approach. Some women in this group have altered gonadal function; others have normal gonadal function and may be candidates for a different pathway.
Postmenopausal women with microprolactinoma often present with mild to moderate prolactin elevation, and the need for active intervention in this subgroup is not always established.
For certain women in this setting, hormone replacement forms part of the management approach. The full protocol specifies when this applies and when a structured observation pathway is appropriate instead.
