Clinical Scenario

This protocol addresses aggressive or malignant prolactinomas that have proven refractory to all standard treatment strategies and for which documented tumor growth has been established. In this setting, systemic chemotherapy becomes the appropriate next step in management.

Treatment Approach

An oral alkylating chemotherapy agent is recommended as first-line chemotherapy following documented tumor growth. The protocol specifies when and how to evaluate early response and sets criteria for determining whether to continue or modify the approach.

For patients with rapid tumor growth and a specific prior radiotherapy history, a combined modality approach incorporating radiotherapy may also apply. The full decision algorithm, response thresholds, continuation criteria, and sequencing details are in the structured protocol.

Aggressive and malignant prolactinomas refractory to all treatment strategies may benefit from the administration of the oral alkylating chemotherapeutic agent temozolomide, whose efficacy in terms of tumor growth control accounts for up to 50% of patients with prolactinomas.
Temozolomide is now recommended as first-line chemotherapy for aggressive pituitary tumors and pituitary carcinomas following documented tumor growth.
The first evaluation of the response to temozolomide should be done after the first 3 cycles.
Conversely, in patients proven to respond to first-line temozolomide after 3 cycles, treatment can be continued for at least 6 months or a longer time if a sustained therapeutic benefit is observed.
The combination of temozolomide and radiotherapy can be offered to patients with rapid tumor growth in whom maximal doses of radiotherapy have not been reached.
