Symptomatic Primary Spontaneous Pneumothorax When Needle Aspiration Has Not Resolved the Pneumothorax

This protocol addresses symptomatic primary spontaneous pneumothorax (PSP) in patients where the initial management line has been attempted but the pneumothorax has not resolved on chest X-ray, indicating the need for an escalated approach.

Prior Treatment & Failure Condition

Initial management of symptomatic PSP involves needle aspiration using a 16-gauge plastic catheter or small-bore pleural catheter, with a possible second aspiration attempt. In centres with appropriate expertise, ambulatory management with a Heimlich (one-way) valve device may be used. Escalation to this protocol is indicated when these measures fail to achieve the treatment goal: resolution of the pneumothorax on chest X-ray.
