Primary Spontaneous Pneumothorax Not Resolved After Conservative Management
Clinical Scenario
This protocol addresses primary spontaneous pneumothorax (PSP) that is minimally symptomatic and clinically and radiologically stable, regardless of pneumothorax size, when initial conservative management has not achieved resolution.
Previous Treatment — Resolution Not Achieved
When conservative management with supplemental oxygen, analgesia, and clinical observation — including confirmation that the patient can walk comfortably — does not result in resolution of the pneumothorax on chest X-ray by 8 weeks, escalation to the next management step is indicated.
Next-Step Approach
This protocol involves a procedural intervention directed at the pleural space. The complete structured regimen is available via the link below.

The panel suggests conservative management of PSP in selected cases (minimally symptomatic and clinically and radiologically stable), regardless of size of pneumothorax.
Insert chest drain and admit.
