Treatment of Primary Sclerosing Cholangitis in Bacterial Cholangitis

Clinical scenario

This protocol addresses patients with primary sclerosing cholangitis (PSC) who develop bacterial cholangitis — a complication arising when biliary strictures allow bacterial infection of the bile ducts. Recurrent episodes are a recognised burden in this population.

Bacterial cholangitis in PSC

Bacterial cholangitis should be treated with antibiotics. In rare cases, patients need to be on rotating antibiotics to prevent recurrent episodes.

Treatment approach — partial overview

In patients with PSC and bacterial cholangitis — particularly when cholangitis is recurrent or reflects advanced disease — a definitive surgical intervention targeting the underlying liver disease is among the treatment options considered.

LT should be considered in all patients with PSC and complications of end-stage liver disease, recurrent cholangitis, intractable pruritus, or early-stage hepatobiliary cancers.
