Treatment of Primary Ovarian Lymphoma in Peripheral T-Cell Lymphoma NOS and Follicular Helper T-Cell Lymphoma
Clinical Scenario
This protocol applies to primary ovarian lymphoma occurring in patients with an underlying diagnosis of peripheral T-cell lymphoma not otherwise specified (PTCL NOS) or follicular helper T-cell lymphoma (TFHL) — two related but distinct T-cell subtypes that shape the treatment approach in relapsed or refractory disease.
Patient Population
The histological subtype — PTCL NOS versus TFHL — is a central determinant of which options apply. Transplant eligibility is an additional factor that influences the management strategy for patients in this setting.
Treatment Approach (Partial Summary)
Management of relapsed or refractory disease in this population may involve salvage chemotherapy regimens and targeted agents, with the specific choice guided by disease subtype and disease characteristics — the complete evidence-graded options and sequencing are available in the full protocol.

- First-line treatment of PTCL not otherwise specified (NOS) or follicular helper T-cell lymphoma (TFHL):
- Options for r/r PTCL NOS and TFHL include 5-azacytidine (for TFHL only), BV (depending on CD30 status), DHAP, GDP, ICE and IVAC-MTX [III, B; 5-azacytidine not FDA or EMA approved].
- For transplant-eligible patients with PTCL or ALCL, consolidation with alloSCT can be considered [II, B].
