Treatment of Primary Ovarian Lymphoma in Gastric Extranodal Marginal Zone Lymphoma

This protocol addresses primary ovarian lymphoma arising in the clinical context of gastric extranodal marginal zone lymphoma (gastric EMZL) — a specific sub-population in which the gastric disease shapes the overall treatment strategy.

Clinical Scenario

The patient has primary ovarian lymphoma in the setting of gastric EMZL. Management of the gastric component is integral to this protocol. Helicobacter pylori (H. pylori) status is a key factor — H. pylori eradication is indicated for all patients with gastric EMZL.

Treatment Approach

Beyond H. pylori eradication, the full protocol defines the conditions under which a targeted locoregional intervention is recommended — based on biopsy findings, symptoms, and other specific clinical indicators. The complete criteria, recommended modality, and sequencing are detailed in the structured protocol.

Helicobacter pylori (H. pylori) eradication treatment should be given to all patients with gastric EMZL [II, A].
If biopsies are positive or if patients are symptomatic or have other treatment indications [e.g. overt progression, deep invasion, nodal involvement, t(11;18)], ISRT can be recommended [III, B].
