Primary ovarian lymphoma presenting as diffuse large B-cell lymphoma (DLBCL) at advanced stage III or IV requires a systemic first-line approach. Treatment selection is guided by patient fitness and individual risk stratification.
This protocol applies to primary ovarian lymphoma in the setting of diffuse large B-cell lymphoma at advanced stage (III–IV). The histological subtype and extent of disease are central to determining the appropriate therapeutic strategy.
For fit patients under 80 years of age, the approach involves rituximab-based immunochemotherapy. The specific combination is selected according to the patient's International Prognostic Index (IPI) score — higher-risk patients receive a different regimen than lower-risk patients.

Advanced stage III-IV:
Fit patients <80 years with an IPI 0-1 should receive six cycles of R-CHOP [I, A].
Six cycles of R-Pola-CHP is recommended as an option for fit patients <80 years with an IPI ≥2 [I, A].
