This protocol addresses primary ovarian lymphoma occurring in the context of anaplastic lymphoma kinase-positive (ALK+) anaplastic large-cell lymphoma (ALCL) — a biologically distinct subtype with specific therapeutic considerations, particularly for relapsed or refractory disease.

- First-line treatment of ALK-positive ALCL:
- For patients with r/r ALK-positive ALCL, BV is recommended if not received as first-line therapy [III, A].
- For patients with ALK-positive ALCL refractory to BV, ALK inhibitors should be considered [III, A]. ChT (e.g. DHAP, ICE or IVAC-MTX) is also an option [III, B].
- For transplant-eligible patients with PTCL or ALCL, consolidation with alloSCT can be considered [II, B].
