Primary Myelofibrosis with Anemia: What to Do After Danazol or Thalidomide Fails
This protocol addresses a specific subset of primary myelofibrosis patients — those with anemia, no deletion 5q [del(5q)], and a serum erythropoietin level of 500 mU/mL or greater — in whom an initial anemia-directed treatment line has not achieved the required response.

Prior Therapy Did Not Achieve Anemia Response
Treatment with Danazol or Thalidomide failed to meet the anemia response threshold: either cessation of transfusions (in transfusion-dependent patients) or a hemoglobin increase of 2 g/dL or greater (in transfusion-independent patients). This failure is the trigger for escalation to the current protocol.
