Primary Erythema Nodosum Not Responding to Potassium Iodide

Clinical scenario

Recurrent or recalcitrant erythematous nodules on the pretibial area persist despite a first-line course of oral potassium iodide. This protocol defines the structured next step when initial management does not achieve clearance.

Previous treatment — goal not reached

Prior therapy: Oral potassium iodide

Target that was not met: Resolution of erythematous nodules on the pretibial area

Failure to reach this endpoint is the escalation trigger for the second-line protocol below.

What the next-line approach involves

Treatment goal: Resolution of recurrent or recalcitrant erythematous nodules on the pretibial area.

Management at this stage draws on oral antimicrobial and anti-inflammatory agents, with corticosteroid therapy reserved for more severe presentations. A localised intervention option also exists for particularly recalcitrant nodules. The full regimen, drug selection criteria, and evidence summary are available in the complete protocol.
