Treatment of Relapsed or Refractory Primary CNS Lymphoma in Fit Patients
Clinical Scenario
This protocol addresses patients with primary CNS lymphoma (PCNSL) whose disease has either relapsed after prior treatment or proved refractory to it, and who remain fit enough to receive intensive salvage polychemotherapy. Early relapse is included in this population.
Treatment Approach
Fit patients in this setting may be candidates for salvage polychemotherapy incorporating high-dose cytarabine-based or high-dose ifosfamide-based combination regimens.

Treatment Goals
- Achievement of response — complete remission or partial response — on brain MRI.
References
- Fit patients with refractory or early relapsed PCNSL can be treated with, for example, HD-ifosfamide- or HD-AraC-based combinations, followed by ASCT or WBRT according to previous treatment.
- HD-AraC- or HD-ifosfamide-based ChT followed by consolidative HDCeASCT is an option for fit patients.
