Treatment of Primary Breast Lymphoma — CD20-Negative, Non-Hodgkin's Type, Stage IE or IIE

Non-Hodgkin's lymphoma pathological type CD20-negative Stage IE or IIE First-line protocol

This protocol covers primary breast lymphoma of non-Hodgkin's histological type in which tumor cells are CD20-negative, presenting at early localised or regionally-involved stage (IE or IIE). Because CD20 expression is absent, anti-CD20 therapy is not applicable — the treatment approach is tailored accordingly.

Management follows a multimodal strategy. Systematic anthracycline-based chemotherapy forms the backbone, delivered over multiple cycles using a specific regimen. This is combined with radiotherapy and, where needed for a pathological diagnosis, limited surgery. The complete regimen, cycle count, radiation field and dose parameters, and surgical specifics are contained in the full protocol.

All patients in this study received systematic chemotherapy after diagnosis; the protocols included the ABVD regimen (comprising doxorubicin, bleomycin, vinblastine, and dacarbazine) (4.5%) for Hodgkin's lymphoma and the CHOP (79.1%) or E-CHOP (16.4%) regimen for non-Hodgkin's lymphoma.

Multiple treatment approaches, including anthracycline-containing chemotherapy, radiotherapy, and limited surgery, are recommended for the initial treatment of patients with PBL.

Rituximab provided a new choice for CD20-positive PBL patients.
