Primary Breast Extra-Nodal Marginal Zone Lymphoma When Rituximab or Radiotherapy Has Not Achieved Complete Response
This protocol addresses patients with primary breast extra-nodal marginal zone lymphoma (PB-MZL) whose first-line therapy has not produced a complete response, and for whom escalation of treatment is now indicated.
Clinical scenario
Primary breast extra-nodal marginal zone lymphoma (PB-MZL) is an indolent subtype of primary breast lymphoma. When the disease does not achieve a complete response with initial treatment, further intervention is required.
Prior treatment & failure condition
First-line therapy for PB-MZL may include rituximab monotherapy continued until complete response, or irradiation and/or rituximab.
This protocol is indicated when the treatment goal of complete response of the breast lymphoma has not been achieved with first-line therapy.
Next-line approach (partial overview)
For PB-MZL that has not adequately responded to initial therapy, the next-line approach involves a combination immunochemotherapy regimen, potentially followed by consolidative radiotherapy. The full protocol details the specific regimen and treatment sequence.

- Primary breast extra-nodal marginal zone lymphoma (PB-MZL) has a similar incidence and prognosis to PB-FL but is more indolent.
- In another patient, 6 months after a low-grade PB-MZL was diagnosed, a high-grade PB-DLBCL developed; R-CHOP and consolidative radiotherapy were then administered, which may not have been required if the PB-MZL had been treated earlier.
