Treatment of Postthrombotic Syndrome Following Upper Extremity Deep Vein Thrombosis

This protocol addresses patients who develop symptoms of post-thrombotic syndrome (PTS) in the upper extremity following an episode of upper extremity deep vein thrombosis (DVT). After upper extremity DVT, a meaningful proportion of patients go on to develop PTS, presenting with persistent arm heaviness, swelling, or discomfort on the affected side.

Patients with symptomatic upper extremity PTS following documented upper extremity DVT. The development of PTS in this setting carries recognised clinical significance, and intervention is considered where symptoms are present.

The initial approach involves a trial of external compression applied to the affected upper extremity. The full protocol specifies the compression parameters, selection criteria, and supporting evidence — access the structured regimen below for the complete recommendation.

After upper extremity DVT, 15–25 % of patients will develop PTS.

Due to potential for benefit and low potential for harm, we suggest a trial of a 20–30 mm Hg or 30–40 mm Hg compression sleeve in patients with symptoms of upper extremity PTS.
