Treatment of Severe or Intractable Postthrombotic Syndrome Symptoms and Edema
Clinical Scenario
Some patients with postthrombotic syndrome experience severe or intractable symptoms — marked limb edema, persistent pain, or skin changes — that are not adequately controlled with standard measures alone. This protocol addresses that more refractory presentation.
Approach — Partial Overview
Dedicated intermittent compression devices — both portable, battery-powered units and clinic-based sleeve systems — may be considered in this setting. The complete protocol covers device selection, application criteria, and clinical decision guidance.

- We suggest that the portable, battery-powered Venowave intermittent compression device be tried in patients with moderate to severe PTS whose symptoms are not adequately controlled with ECS alone.
- We suggest that intermittent pneumatic compression sleeve units (e.g. used for 20–30 min sessions, 2–3 times per day) can be used to help severe, intractable PTS symptoms or severe edema, however patients may find these to be cumbersome and the units are expensive.
