Treatment of Posterior Ischemic Optic Neuropathy Due to Non-Arteritic Causes

This protocol targets the management of posterior ischemic optic neuropathy (PION) in patients where the underlying cause is not giant cell arteritis — the non-arteritic form known as NA-PION. The cause of the ischemic insult and the stage at which the patient presents both bear on clinical decision-making.

Clinical Scenario

Non-arteritic PION (NA-PION): posterior ischemic optic neuropathy due to causes other than giant cell arteritis (GCA). This presentation is distinct from arteritic PION and determines the relevant treatment pathway.

Treatment Approach

The protocol centres on systemic steroid therapy initiated at a specific point early in the disease course — the precise regimen, parameters, and decision criteria are detailed in the full protocol.

Treatment targets: improvement in visual acuity and visual field.

Non-arteritic PION (NA-PION): due to causes other than GCA.

NA-PION patients, when treated with high-dose systemic steroid therapy during the very early stages of the disease, showed significant improvement in visual acuity and visual fields, compared to untreated eyes.
