Post-herpetic neuralgia (PHN) is persistent neuropathic pain that continues after a herpes zoster episode. Establishing an appropriate first-line pharmacological strategy is the central decision in early management.
First-line management is based on pharmacological monotherapy. The approach draws from a small set of well-established drug classes for neuropathic pain — with the choice between them guided by pain severity, affected body area, and individual patient factors. A topical option is also recognised as first-line in certain presentations.
