Treatment of Post COVID-19 Condition in Long COVID with Postural Tachycardia Syndrome

A subset of long COVID patients develop postural orthostatic tachycardia syndrome (POTS), an autonomic complication that shapes the management of Post COVID-19 condition and calls for a targeted therapeutic approach based on the patient's hemodynamic status.

Clinical scenario
Long COVID with POTS symptoms. When patients with long COVID complain of POTS symptoms, both non-pharmacological and pharmacological treatments may be considered depending on their individual hemodynamic status.
First-line approach
For this scenario, a structured non-pharmacological treatment program is the initial management strategy — addressing fluid balance, physical conditioning, and specific supportive measures. The full protocol specifies the complete set of interventions and the criteria that guide escalation beyond first-line care.

When patients with long COVID complain of POTS symptoms, non-pharmacological and pharmacological treatments may be considered depending on their hemodynamic status (G, IIb).
Upon diagnosis of POTS, non-pharmacological treatment is first considered (Table 7).
