This protocol is for patients with long COVID who experience discomfort after exercise — a presentation referred to as post-exertional malaise (PEM) or post-exertional symptom exacerbation (PESE). Management in this specific group requires careful attention to how activity is approached.
In long COVID patients who experience PEM, the approach to activity is central. The protocol defines when and how activity adjustments are warranted, and what clinicians should avoid in this population.

In cases where there is discomfort after exercise in patients with long COVID, educating on the importance of alternating between appropriate activity and rest may be helpful (G, IIb).
Clinicians should not force patients experiencing PEM/PESE to increase their activity levels and should instead encourage them to stop working and rest.
