Fatigue is among the most common and persistent symptoms reported in long COVID. In patients with post-COVID condition where fatigue is the dominant concern, clinical management begins with identifying any correctable underlying cause — and follows a structured rehabilitation pathway when none is found.
This protocol addresses long COVID patients presenting with fatigue. Evaluation first focuses on correcting any identifiable underlying cause of fatigue. Where no specific organic cause is established, a rehabilitation-based approach becomes the recommended pathway.
When no specific underlying cause of fatigue is identified, the evidence-based management centres on rehabilitation therapy — a structured programme encompassing physical recovery modalities adapted to the patient's functional capacity.

For fatigue symptoms in patients with long COVID, correctional therapy of the underlying causes is necessary. If no specific underlying causes exist, rehabilitation therapy may be considered (G, IIb).
For patients where no other organic cause of fatigue is identified, rehabilitation treatments such as muscle strengthening, stretching, balance training, gait training, aquatic therapy, yoga, and physical therapy can be considered.
