Popliteal Artery Entrapment Syndrome with Minimal Arterial Damage and No Occlusion or Stenosis
Clinical Scenario
This protocol addresses popliteal artery entrapment syndrome (PAES) in the specific situation where arterial damage is minimal and there is no occlusion or stenosis of the popliteal artery. In this presentation, the artery remains patent but is subjected to external compression by abnormal musculotendinous structures.
Surgical Approach
When arterial damage is minimal, the surgical strategy targets release of the compressing musculotendinous structure — a focused decompression procedure — rather than arterial reconstruction. The complete technique, including approach, extent of resection, and decision criteria, is detailed in the full protocol.
Primary goal: freedom from symptoms.

- If the arterial damage is minimal, myotomy of the MHGM or any abnormal musculotendinous slips may suffice.
- The total resection of the abnormal musculotendinous slips might be more effective to be free from the compression.
- The primary objective of surgical treatment for PAES is to free the patient from symptoms.
