Treatment of PCOS with Hirsutism and Irregular Menstrual Cycles in Reproductive-Age Adults
Polycystic ovarian syndrome (PCOS) in reproductive-age adults often presents with hirsutism and/or irregular menstrual cycles. These features require a structured, evidence-based approach that addresses both androgen-driven changes and cycle irregularity.
Clinical Scenario
This protocol targets reproductive-age adults with PCOS who present with hirsutism (excess hair growth with an androgen-related pattern) and/or irregular menstrual cycles. Management addresses both manifestations within a single, coordinated plan.
Treatment Overview (Partial)
The approach involves anti-androgen therapy used in combination with effective contraception, with additional consideration of mechanical interventions specifically targeting facial hirsutism. The complete regimen — including therapy selection criteria, sequencing, and full options — is available in the structured protocol below.

Combined oral contraceptive pills (COCP) could be recommended in reproductive age adults with PCOS for management of hirsutism and/or irregular menstrual cycles.
In combination with effective contraception, anti-androgens could be considered to treat hirsutism in women with PCOS, if there is a suboptimal response after a minimum of six months of COCP and/or cosmetic therapy.
Mechanical laser and light therapies should be considered for reducing facial hirsutism and for related depression, anxiety, and quality of life in women with PCOS.
