What Is the Treatment of Newly Diagnosed Pleural Mesothelioma with Epithelioid Histology?
This protocol covers systemic treatment decisions for patients at first presentation with pleural mesothelioma confirmed to carry an epithelioid histologic subtype — the most common and generally more treatment-responsive subtype.
Clinical Scenario
Patients with newly diagnosed epithelioid pleural mesothelioma have access to established first-line systemic treatment options. Immunotherapy-based regimens are a recognised first-line approach, as is pemetrexed plus platinum-based chemotherapy, with or without an additional targeted agent. The choice between these strategies depends on the individual clinical context.

- In patients with newly diagnosed epithelioid mesothelioma, ipilimumab plus nivolumab immunotherapy should be offered as a first-line systemic treatment option.
- In patients with epithelioid histology, pemetrexed plus platinum-based chemotherapy with or without bevacizumab may be offered as a first-line systemic treatment option.
- In patients who have received both immunotherapy and pemetrexed plus platinum-based chemotherapy, vinorelbine or gemcitabine ± ramucirumab may be offered.
- Retreatment with pemetrexed-based chemotherapy may be offered to patients with pleural mesothelioma who achieved durable (>6 months) disease control with first-line pemetrexed-based chemotherapy.
