Second-Line Treatment for Mild Pemphigus Foliaceus When First-Line Therapy Has Not Achieved Disease Control
Clinical Scenario
This protocol addresses patients with pemphigus foliaceus in a mild disease state — defined as involved body surface area (BSA) below 5% and/or a Pemphigus Disease Area Index (PDAI) score of 15 or less — in whom first-line therapy has not resulted in disease control.
When First-Line Therapy Falls Short
First-line options for mild pemphigus foliaceus include dapsone, topical corticosteroids, systemic corticosteroids, and rituximab. When these measures fail to achieve disease control, escalation to a structured second-line approach is indicated.

- PF with involved body surface area (BSA) <5%
- PDAI score ≤ 15.
- Rituximab: two infusions of 1g two weeks apart, alone, or associated with topical corticosteroids or oral prednisone 0.5 mg/kg/day, in patients previously treated with dapsone or topical corticosteroids
- Systemic corticosteroids: prednisone 0.5–1.0 mg/kg/day with or without azathioprine (1 to 2.5 mg/kg/d), or mycophenolate mofetil 2g/day or mycophenolate sodium 1440 mg/d – if rituximab is not available or contraindicated
