Treatment of Neuropathic Pain in Parkinson's Disease

Pain in Parkinson's disease is not uniform. When the pain is specifically neuropathic in character — and nociceptive pain has been excluded — a targeted pharmacological protocol distinct from general pain management applies.

Clinical Scenario

This protocol is for patients with Parkinson's disease whose pain is confirmed to be neuropathic in nature. It does not apply where the pain is nociceptive. Accurate pain-type classification is the prerequisite for this approach.

Treatment Approach — Partial Overview

Management is pharmacological, involving an antidepressant-class agent as part of a combination strategy. The full combination — including the additional medication class, sequencing, and evidence-based selection criteria — is detailed in the complete structured protocol.

Neuropathic pain should be treated with anticonvulsants and/or antidepressants following the guideline for neuropathic pain treatment, with a preference for Gabapentin and/or Duloxetine (especially in comorbidity with depression).
