Treatment of Parkinson's Disease with Urinary Urgency and Bladder Detrusor Overactivity

Clinical scenario

This protocol applies to patients with Parkinson's disease who experience lower urinary tract symptoms — specifically increased urinary frequency, urinary urgency, and bladder detrusor overactivity. These are recognised autonomic manifestations that require targeted management alongside the primary neurological condition.

Presenting features

The case is defined by the following urinary symptoms in the context of Parkinson's disease:

Increased urinary frequency Urinary urgency Bladder detrusor overactivity
Treatment approach (partial)

The protocol involves a beta-3 receptor agonist class of pharmacological intervention targeting urinary urgency and incontinence arising from bladder detrusor overactivity. Full eligibility criteria, treatment sequence, and regimen details are available in the complete protocol.

References

Antimuscarinics such as Solifenacin, Trospium or Darifenacin can be considered for treatment of increased urinary frequency and urgency due to overactivity of the bladder detrusor in PD patients.

A treatment with β3-receptor agonists (e.g., Mirabegron) for both urinary urgency and incontinence due to bladder detrusor overactivity may be considered for patients who have not adequately responded to antimuscarinics, cannot tolerate them, or have contraindications to them.
