Parathyroid carcinoma may present in a locally advanced state or with distant spread. This protocol addresses the management of patients whose disease has progressed beyond surgical cure, including those with metastases to distant organs.
This protocol applies to patients with locally advanced parathyroid carcinoma or confirmed distant metastases. Sites of distant spread include:
Parathyroid carcinoma is often locally advanced at presentation, and distant metastases can occur in the lung (40%), bone (30%), and liver (10%).
PC is often locally advanced and distant metastases can occur in the lung (40%), bone (30%) and liver (10%).
The efficacy of systemic treatment in advanced and metastatic disease is limited; thus, whenever possible, debulking surgery and/or locoregional treatments should be discussed to control tumour burden and hormonal secretion [V, A].
Cinacalcet should be continued in case of persistent severe hypercalcaemia.
