Parathyroid Adenoma: Next-Line Approach When Minimally Invasive Parathyroidectomy Did Not Achieve Eucalcemia

For patients with parathyroid adenoma in whom a focused initial surgical procedure failed to restore lasting normal calcium levels, a structured next-line protocol guides the appropriate escalation in management.

Image-guided focused minimally invasive parathyroidectomy (MIP) with intraoperative parathyroid hormone monitoring (IPM) was performed as the initial surgical approach. The required targets — eucalcemia lasting a minimum of 6 months and normalization of parathyroid hormone levels at 6 months — were not achieved, prompting escalation to a next-line protocol.
