Parathyroid Adenoma: Next-Line Approach When Minimally Invasive Parathyroidectomy Did Not Achieve Eucalcemia

Prior Treatment — Goals Not Met

Image-guided focused minimally invasive parathyroidectomy (MIP) with intraoperative parathyroid hormone monitoring (IPM) was performed as the initial surgical approach. The required targets — eucalcemia lasting a minimum of 6 months and normalization of parathyroid hormone levels at 6 months — were not achieved, prompting escalation to a next-line protocol.

Next-Line Surgical Strategy

When intraoperative monitoring during MIP indicates that resection was not complete, the next step involves a more comprehensive operative approach to the parathyroid glands — the full protocol specifies the technique, surgical landmarks, and decision criteria.

During MIP, if IPM suggests residual hypersecreting tissue, conversion to bilateral exploration (BE) is warranted and has equivalent cure rates to planned BE.
Bilateral exploration is defined by a standard technique in which all parathyroid glands are identified with exploration of expected and, if necessary, ectopic cervical locations.
Cure after parathyroidectomy is defined as the reestablishment of normal calcium homeostasis lasting a minimum of 6 months.
