Parapharyngeal Abscess After Clindamycin Failure in a Patient With Previous Anaphylactic Reaction to Penicillin

This protocol applies to patients with parapharyngeal abscess who have a documented prior anaphylactic reaction to penicillin and whose initial antibiotic regimen has not produced the expected clinical response.

The patient has a history of previous anaphylactic reaction to penicillin. Patients in this setting may be treated with clindamycin as an alternative antibiotic approach.

Initial management for this population includes airway evaluation and stabilisation, hydration, analgesia, and parenteral high-dose clindamycin. This next-step protocol is indicated when clinical improvement has not been achieved within 48 hours of initiating intravenous antibiotic therapy.

When antibiotic therapy has not produced adequate clinical improvement, a surgical intervention becomes the central component of management — and this protocol specifies that it must be preceded by careful assessment of airway patency.
