Paracoccidioidomycosis with 5–10% Weight Loss: Next-Line Protocol When Oral Therapy Does Not Achieve Treatment Goals

This protocol addresses moderate paracoccidioidomycosis presenting with total body weight loss between 5 and 10% and counter immunoelectrophoresis titers between 1/16 and 1/32 — in a patient who has not achieved the expected treatment targets on a standard oral regimen.

Clinical Presentation

The moderate form of PCM is characterised by total body weight loss between 5 and 10% and counter immunoelectrophoresis titers in the range of 1/16 to 1/32. These parameters distinguish it from both mild and severe presentations.

Previous Treatment — Goals Not Reached

A prior two-phase oral therapy course was undertaken. The expected endpoints — resolution of signs and symptoms, normalization of inflammatory markers, and a meaningful decrease in antibody titers — were not achieved. This failure of the oral approach is the basis for escalation to the protocol below.

Next-Line Approach
