Management of Unifocal Papillary Thyroid Microcarcinoma ≤10 mm Without Extracapsular Extension or Lymph Node Metastases

This protocol covers the management of a well-defined low-risk presentation of papillary thyroid cancer: a unifocal microcarcinoma measuring ≤10 mm, with no extracapsular extension and no lymph node involvement.

Clinical Scenario

Unifocal papillary thyroid microcarcinoma measuring ≤10 mm, with confirmed absence of extracapsular extension and no evidence of lymph node metastases.

Approach Overview

For this low-risk stage, the protocol centres on an imaging-based monitoring strategy of the thyroid and cervical lymph nodes, with defined criteria for when a surgical alternative is considered instead.

The complete protocol specifies monitoring intervals, precise surgical indications, and further management steps.
Treatment Goal

No significant tumour growth (≥3 mm) and no onset of lymph node metastasis.

References

Active US surveillance of the thyroid and neck lymph nodes (every 6–12 months) can be proposed for unifocal papillary microcarcinomas (10 mm) with no evidence of extracapsular extension or lymph node metastases.

Lobectomy (instead of total thyroidectomy) may be proposed for selected low-risk (T1a–T1b–T2, N0) tumours.
