This protocol covers the management of a well-defined low-risk presentation of papillary thyroid cancer: a unifocal microcarcinoma measuring ≤10 mm, with no extracapsular extension and no lymph node involvement.
Unifocal papillary thyroid microcarcinoma measuring ≤10 mm, with confirmed absence of extracapsular extension and no evidence of lymph node metastases.
No significant tumour growth (≥3 mm) and no onset of lymph node metastasis.
Active US surveillance of the thyroid and neck lymph nodes (every 6–12 months) can be proposed for unifocal papillary microcarcinomas (10 mm) with no evidence of extracapsular extension or lymph node metastases.
Lobectomy (instead of total thyroidectomy) may be proposed for selected low-risk (T1a–T1b–T2, N0) tumours.
