Treatment of Pancreatic Pseudocyst Located in the Uncinate Process with Pancreatic Duct Communication

This protocol addresses a pancreatic pseudocyst situated specifically in the uncinate process of the pancreas, where a communication has been identified between the pseudocyst and the main pancreatic duct. This anatomical configuration requires careful evaluation of the ductal relationship before selecting a management strategy.

When the pseudocyst is located in the uncinate process, the relationship between the pseudocyst and the pancreatic duct must be established. The presence of a duct–pseudocyst communication directly influences the choice of drainage approach and the operative planning.

Management in this setting involves surgical internal drainage. The specific operative procedure selected depends on the anatomical findings at the time of intervention.

When the pancreatic pseudocyst is located in the uncinate process of the pancreas, the relationship between the pseudocyst and the pancreatic duct must be evaluated.

If a communication exists between them, endoscopic drainage via a transpapillary approach can be achieved.

The operative procedures performed were Roux-en-Y cystojejunostomy in 21 patients (42.9%), cystoduodenostomy in 15 patients (30.6%), cystogastrostomy in 7 patients (14.3%), and distal pancreatectomy in 6 patients (12.2%).
