Treatment of Metastatic Pancreatic Cancer with ECOG Performance Status 3–4

Patients presenting with metastatic pancreatic cancer and a markedly reduced functional capacity — defined as ECOG performance status 3 or 4 — represent a distinct clinical population requiring careful individualisation of care.

Clinical scenario: Metastatic pancreatic cancer in a patient with ECOG performance status 3–4 (largely confined to bed or chair; limited self-care). At this level of performance impairment, the balance of benefit and harm from systemic therapy shifts substantially.

Approach in this setting

In this setting, the evidence-based approach centres on symptom-directed care — the full structured regimen, including specific interventions and sequencing, is available via the link below.

For patients with ECOG PS 3-4, symptom-directed care should be considered, as the risks of any ChT likely outweigh any benefit in this setting [IV, A].
