Patients presenting with metastatic pancreatic cancer and a markedly reduced functional capacity — defined as ECOG performance status 3 or 4 — represent a distinct clinical population requiring careful individualisation of care.
Clinical scenario: Metastatic pancreatic cancer in a patient with ECOG performance status 3–4 (largely confined to bed or chair; limited self-care). At this level of performance impairment, the balance of benefit and harm from systemic therapy shifts substantially.
