First-Line Treatment of Metastatic Pancreatic Cancer with ECOG Performance Status 2
Clinical Scenario
This protocol is for patients with metastatic pancreatic cancer who present with a reduced but not severely impaired functional capacity — an ECOG performance status of 2 — while retaining sufficient physiological reserve and adequate hepatic function to receive active systemic treatment.
Patient Eligibility Criteria
- Metastatic pancreatic cancer (stage IV)
- ECOG PS 2 reduced functional status
- Karnofsky performance status ≥70
- Serum bilirubin ≤1.5× upper limit of normal
Treatment Approach (overview only)
For patients meeting these criteria, first-line systemic chemotherapy is an evidence-supported option. The complete regimen, dosing schedule, and sequencing algorithm are available in the full structured protocol.

Monitoring & Treatment Goals
Treatment efficacy is evaluated every 8–12 weeks, based on clinical status, CA 19-9 trajectory, and imaging findings.

- For patients with ECOG PS 2, Karnofsky PS (KPS) 70 and bilirubin level 1.5 times the ULN, GN can be considered [II, A; ESMO-MCBS v1.1 score: 3].
- The efficacy of treatment should be typically evaluated every 8-12 weeks and should be based on clinical status, CA 19-9 trajectory and imaging [III, A].
